Declaration of Eligibility:

1. I, the undersigned, declare my eligibility for the SASSA (South African Social Security Agency) SRD (Social Relief of Distress) grant, and I affirm that:

I am a South African citizen, Permanent Resident, or a registered Refugee on R350 the Home Affairs database, or I hold a special permit under specific dispensations, including the Special Angolan Dispensation, Lesotho Exemption Permit dispensation, or Zimbabwe Exemption Permit Dispensation. I may also be an asylum seeker with a valid section 22 permit or visa as of 15 March 2020.
3. I currently reside within the borders of the Republic of South Africa.
4. I am above the age of 18 and below the age of 60.
5. I am unemployed.
6. I am not receiving any adult social grant for myself.
7. I am not receiving an unemployment insurance benefit and do not qualify for one.
8. I am not receiving an unemployment insurance benefit and do not qualify for one.
9. I am not receiving a stipend from the National Student Financial Aid Scheme or other forms of financial aid.
10. I am not receiving any other government COVID-19 response support, and R350 .
11. I am not a resident in a government-funded or subsidized institution.
12. I confirm that all information, including Personal Information, provided to SASSA is valid, accurate, complete, and current.
13. I undertake to immediately notify SASSA of any changes in my financial circumstances, contact details, or circumstances that would disqualify me from receiving the COVID-19 special relief of distress grant as per clause 1.1.
14. I also agree to correct and update my Personal Information if any changes occur.
15. I confirm that the provided mobile phone number is accurate and the acceptable channel for all communication from SASSA regarding this grant. I commit to updating this number with SASSA if it changes.
16. I understand that providing false or misleading information is punishable by law, and such information may justify the denial or revocation of my grant application by SASSA.

Personal Information Processing:

• I understand that Personal R350  Information, as defined by the Protection of Personal Information Act, Act No. 4 of 2013 (POPI Act), pertains to me as an identifiable natural person and includes all information supplied to SASSA regarding my application.
• I acknowledge that SASSA will process my Personal Information in compliance with the POPI Act and all other applicable legislation.

Voluntary Provision of Personal Information:

Awareness of Grant Processes:

• I consent to Institutions processing, sharing, transferring, and verifying my Personal Information between Institutions and SASSA for the same purpose.

Data Retention:

• I understand that SASSA and Institutions will retain my Personal Information for as long as needed to achieve the specified purpose or as required by law, whichever is longer.

Rights and Complaints:

• I acknowledge my right to request access to, correction of, and deletion of my Personal Information. I understand that I may complain to the Information Regulator and withdraw consent, which will result in the cancellation of my grant application.

Data Privacy:

• I understand that privacy is essential to SASSA and Institutions. They will use reasonable efforts to ensure that Personal Information is kept confidential, stored securely, and processed in compliance with the POPI Act. Unconditional Consent:

• By submitting my Personal Information to SASSA, I provide reasonable, unconditional, specific, and voluntary written consent for the processing, sharing, transferring, and verification of such Personal Information by SASSA and Institutions for the specified purposes.
• These purposes may include disclosing taxpayer information to the South African Revenue Service (SARS), verifying information against various databases, and confirming financial status with commercial banks registered in South Africa, among others.
• I understand that SASSA may decline my grant based on findings by the Auditor General of South Africa (AGSA) and may request information held by AGSA for verification purposes.
•  R350 SASSA may also share my Personal Information for auditing, planning, and economic development opportunities.
